A great meeting starter. Kick off meetings by copying and handing out “The facts” page of a case that fits your situation. Discuss the scenario with your team, solicit opinions about what your supervisors think happened. Then, discuss the actual outcome and critical safety lesson to be learned.
Practical insights for those dealing with a specific problem. If a supervisor is struggling with a certain safety issue, say lockout/tagout for example, give him or her the entire section of cases to study. There’s no better way to drive home a message than to learn from other people’s actual mistakes.
